چکیده

Osteoarthritis of the hip has a greatly varied incidence in different parts of the world. In Britain it has for many years presented one of the commonest and biggest problems in orthopaedic surgery-a problem which had shown little sign of solution until the introduction of intertrochanteric osteotomy by McMurray (1935). This operation entailed prolonged immobilisation, which excluded many elderly patients until the introduction of effective methods of internal fixation. More recently the total prosthesis, which may yet undergo many changes, has enabled us to deal remarkably effectively with the grossly damaged and distorted hip. In the face of two operations of such obvious value as these it is easy to forget older operations of proved worth. Pseudarthrosis as described by Girdlestone (1945) and modified by Batchelor (1945) and Milch (1948) will always remain as a stand-by and last resort. Arthrodesis has become an uncommon operation confined almost entirely to young people. Arthrodesis has been criticised on the grounds that it involves a prolonged period of

Reason and treatment of failure of proximal femoral nail antirotation internal fixation for femoral intertrochanteric fractures of senile patients.

The cause of postoperative failure after the treatment of femoral intertrochanteric fracture with proximal femoral nail antirotation (PFNA) was analyzed, and the reoperative methods were examined. Nine failures of 308 femoral intertrochanteric fracture patients with PFNA were treated with femoral prosthesis total hip replacement (THR) and reoperative internal fixation. All nine patients were an...

Proximal Femoral Nail versus Dynamic Hip Screw Fixation for Trochanteric Fractures: A Meta-Analysis of Randomized Controlled Trials

BACKGROUND The purpose of this meta-analysis was to find out whether the proximal femoral nail was better than the dynamic hip screw in the treatment of trochanteric fractures with respect to operation time, blood transfusion, hospital stay, wound complications, number of reoperation, and mortality rate. METHODS All randomized controlled trials comparing proximal femoral nail and dynamic hip ...

Clinical Outcomes of Intramedullary and Extramedullary Fixation in Unstable Intertrochanteric Fractures: A Randomized Clinical Trial

Background: The best method for repairing intertrochanteric fractures is still controversial. The fixation methods includeextramedullary (EM) and intramedullary (IM). Studies that compare IM and EM fixations for unstable hip fractures arerare. In this study, our goal was to compare the efficacy of EM and IM fixation in treatment of unstable intertrochantericfractures.Met...

Gamma and other cephalocondylic intramedullary nails versus extramedullary implants for extracapsular hip fractures in adults.

BACKGROUND Two types of implants used for the surgical fixation of extracapsular hip fractures are cephalocondylic intramedullary nails, which are inserted into the femoral canal proximally to distally across the fracture, and extramedullary implants (e.g. the sliding hip screw). OBJECTIVES To compare cephalocondylic intramedullary nails with extramedullary implants for extracapsular hip frac...

Is a sliding hip screw or im nail the preferred implant for intertrochanteric fracture fixation?

UNLABELLED This study was performed to determine whether patients who sustain an intertrochanteric fracture have better outcomes when stabilized using a sliding hip screw or an intramedullary nail. A 20% sample of Part A and B entitled Medicare beneficiaries 65 years or older was used to generate a cohort of patients who sustained intertrochanteric femur fractures between 1999 and 2001. Two fra...

Comparison of Outcome of Femoral Shaft Fracture Fixation with Intramedullary Nail in Elderly Patient and Patients Younger than 60 Years Old

Background:   Although intramedullary nailing (IMN) is an established and accepted operative treatment for femoral shaft fracture in patients younger than 60, there is a lack of data on the results of this treatment on those over 60. The purpose of this study was to determine if the outcome of IMN for femoral shaft fracture in elderly patients is also acceptable. Particular challenges in this g...
